Creating a Custom Form

Create a new custom report template using Word to meet your specific requirements.

There are two ways to create a custom report template using Word.

  • Download an existing sample custom report template.
  • Create a custom report template from scratch.

The easiest method is to download an existing custom report template. Then you only need to make the changes you want, save it and upload it as a new custom report.



  • The interface in this video is not the interface I have  for Report Builder in SD 2.0

  • The interface in not the same. If you have a Mac the word "merge field" portion is not described correctly. The templates do not load into the program. The reports do not save to the reports.
